What UTAS asks for
UTAS has no practitioner section. The certificate carries the doctor's opinion, so what it states is what decides the outcome.
What the same evidence is for
Students search for these by name. UTAS treats them as one process with different outcomes.
This applies where you sat the examination but illness or other circumstances disadvantaged your performance. The form asks your lecturer to take the circumstances into account when marking. It goes to the relevant school with your supporting documentation attached.
Where illness stopped you sitting the exam at all, a deferred examination is the right application rather than special consideration in marking. UTAS is explicit that a certificate does not guarantee approval, so the documentation needs to show the impact clearly.
For coursework rather than exams, an extension moves the submission date. It is handled by your unit coordinator or school rather than through the exams forms page, and a dated certificate covering the due date supports the request.

See a doctor as close as you can to the examination date, so the certificate covers the day the exam fell on.
Tell the doctor which examination was affected and whether you sat it or missed it, because UTAS uses different forms for each.
Download the Application for Special Consideration in the Marking of Exams, or the deferred examination form, from the Exam forms page in the Student Portal (linked from UTAS Ask Us).
Complete the form and attach the certificate, checking it carries the consultation date, the period covered, and the practitioner's registration details.
Submit it directly to the relevant school as soon as possible after the examination, rather than waiting for results to be released.

UTAS's published exam forms material does not address consultation modality. It asks for appropriate supporting documentation with the form. Since the University neither permits nor excludes telehealth, a certificate backed by a real consultation is the safer choice.
UTAS asks for forms to be submitted as soon as possible after the completion of the examination, rather than publishing a fixed number of days. In practice that means before marking is finalised, so lodge it without waiting for your result.
