What UniMelb asks for

This is the main application when illness affects an assessment, an exam or your attendance. You lodge it through my.unimelb within four business days of the due date and attach the Health Professional Report. The outcome depends on the severity ratings your practitioner records.
An extension is a short move of a submission date and is one of the outcomes a special consideration application can produce. It suits a brief illness that delayed a piece of work rather than one that affected your performance in it.
Where you could not sit an exam or timed assessment, the outcome can be an alternative or rescheduled sitting. The Health Professional Report needs to show the impact fell on the assessment date, with the attendance domain rated.
If the circumstances will affect your study for longer than six weeks, the Health Professional Report is the wrong instrument. UniMelb directs you to register for ongoing support, which sets up adjustments across the semester instead of one assessment at a time.

Book a consultation, in person or by telehealth, while the condition is affecting you. UniMelb treats both modalities the same.
Bring the Health Professional Report to the consultation and tell the practitioner which assessments and dates were affected.
Ask the practitioner to rate impact in each of the three domains and to confirm the period of impact is six weeks or less. They should not write a diagnosis.
Collect the completed report in an uneditable PDF, signed, with the practitioner's registration body and number.
Lodge the special consideration application through my.unimelb and upload the report within four business days of the assessment due date.

The practitioner rates the severity of impact on administrative tasks, academic work and attendance, records the consultation dates and period of impact, and signs with their registration body and number. It replaces a plain medical certificate.

No. UniMelb does not want the condition named. The form asks for the effect on your capacity to study and attend, not the underlying diagnosis, which keeps the clinical detail with your practitioner.
