What WSU asks for
WSU has no practitioner section. The certificate carries the doctor's opinion, so what it states is what decides the outcome.
What the same evidence is for
Students search for these by name. WSU treats them as one process with different outcomes.
Special consideration covers compassionate and compelling circumstances that affected an assessment task. Apply within five working days of the due date, the missed activity, or the start of your circumstances. Independent supporting documentation goes in with the application.
An extension moves the due date on an assessment task. Western Sydney asks for the request within the same five working day window and, for later requests in a subject, with supporting documentation attached. A dated certificate naming the period you were affected is the simplest evidence.
Compulsory classes, labs and placements have their own attendance requirements, and illness that stops you attending is handled through the same application. The certificate needs to name the dates of the classes you missed. Apply within five working days of the missed activity.
If illness stopped you sitting an exam, apply for special consideration and ask for a deferred exam. The medical documentation needs enough detail for the severity of the condition to be assessed against the exam date. A statutory declaration is only accepted where no other document is available.

See a doctor while the illness is current, so the certificate covers the assessment date rather than being written afterwards.
Ask for detail, not just dates. Western Sydney needs enough information to work out the severity of the condition in relation to your assessment.
Check the document is signed, dated, and on letterhead where the practice uses it.
Lodge the Special Consideration Application Form through the University's service portal and attach the documentation.
Apply within five working days of the due date, the missed compulsory activity, or the start of your circumstances.
If you are lodging late, explain why and make sure the documentation covers that period too.

Western Sydney's published rules do not address how the consultation happened. They ask for independent documentation, signed and dated, with enough detail for the severity of the condition to be reasonably ascertained. Silence on modality is not the same as acceptance, so make sure the certificate carries real clinical detail.
Within five working days of the assessment due date, the missed compulsory teaching activity, or the date your circumstances began. Late applications are possible, but your supporting documentation then has to cover the late submission as well.
It needs enough medical detail that the severity and gravity of the condition can be reasonably ascertained in relation to your teaching, learning or assessment. It must be independent, signed and dated. A one line note that you were unwell does not meet that standard.
Only as a last resort. Western Sydney says a statutory declaration may be considered for special consideration or a deferred exam where no other supporting document is available. A certificate from a practitioner is the stronger evidence.
Western Sydney publishes an exception for the first extension request in a subject. Every other application, including later extensions in the same subject, needs supporting documentation lodged with it.
