What Macquarie asks for
Macquarie's practitioner form is specific. These are the items the doctor completes after assessing you.

Students search for these by name. Macquarie treats them as one process with different outcomes.
Special consideration covers serious, unavoidable and significantly disruptive circumstances that affected an assessment task. You lodge through Service Connect within five working days of the due date and attach the Professional Authority Form. Macquarie aims to communicate the outcome within five working days of receiving the application and all the evidence.
A supplementary exam is one of the outcomes Macquarie can grant when illness affected a final exam. Other than in exceptional circumstances, only one supplementary exam opportunity is given. The Professional Authority Form must show you were unfit on the original exam date.
Where the affected task is not an exam, the usual outcome is an extension. Macquarie normally sets the length of the extension to match the duration of the circumstances, so the dates on the form matter. Short extensions on some tasks are handled separately from a full application.
Where the circumstances are serious but no alternative assessment is possible, Macquarie may recommend that you apply to withdraw without penalty. That is a separate application, and you decide whether to take it. The Professional Authority Form has a tick box for the withdrawal case.
How to apply at Macquarie
See a doctor while you are unwell and bring the Professional Authority Form with you.
Have the doctor rate you as fit, limited, unfit or not assessable, and record whether the condition is short term, chronic, or an exacerbation.
Check the special consideration or withdrawal tick matches your application, and that your name, date of birth and signature are on the student section.
Lodge the application online through Service Connect and attach the completed form.
Submit no later than five working days after the assessment task due date.
Keep the original document for six months. Macquarie can ask for originals and expects them within ten working days.

The Professional Authority Form, or PAF, is Macquarie's form for documenting medical disruptions, physical or psychological. The practitioner rates you as fit, limited, unfit or unable to assess, records whether the condition is short term, chronic or an exacerbation, and signs and stamps it. You complete and sign the student section.
