The Professional Authority Form, or PAF, is Macquarie's form for documenting medical disruptions, physical or psychological. The practitioner rates you as fit, limited, unfit or unable to assess, records whether the condition is short term, chronic or an exacerbation, and signs and stamps it. You complete and sign the student section.

No. The procedure states that once you have submitted an application you may not withdraw it, and you must accept the outcome as determined. Check the Professional Authority Form is complete and ticked for the right process before you lodge.
It can. A supplementary examination is one of the listed outcomes, and other than in exceptional circumstances only one opportunity is provided. The other outcomes are an extension matched to the duration of your circumstances, or a recommendation to apply to withdraw without penalty.

Special consideration covers serious, unavoidable and significantly disruptive circumstances that affected an assessment task. You lodge through Service Connect within five working days of the due date and attach the Professional Authority Form.
A supplementary exam is one of the outcomes Macquarie can grant when illness affected a final exam. Other than in exceptional circumstances, only one supplementary exam opportunity is given.
Where the affected task is not an exam, the usual outcome is an extension. Macquarie normally sets the length of the extension to match the duration of the circumstances, so the dates on the form matter.
