UTS states that it currently accepts medical certificates issued by a registered health practitioner after a face-to-face, video conferencing, or telephone consultation. The modality list is the condition: the certificate has to follow a consultation with the practitioner.

What UTS asks for

What the same evidence is for
Students search for these by name. UTS treats them as one process with different outcomes.
Use this when illness affected an exam you sat, or stopped you attending one. For a centrally conducted exam you attended, UTS asks for the application within 48 hours of the exam, excluding weekends. If you know in advance that you cannot attend a faculty exam or quiz, apply at least ten working days before it.
A long extension moves the due date on an assessment task and is lodged before the original due date. The Professional Authority Form should state the start and end dates of the impact so the faculty can match the extension to the period you were affected.
If illness affected work you submitted or could not submit, apply for special consideration on that task. UTS asks the practitioner to rate how severely your ability to complete the task was affected. The outcome is decided by the faculty, not by the certificate alone.

Check the form carries both the AHPRA registration number and the Medicare provider number, plus the practitioner's signature and contact details.
Log in to My Student Portal with your UTS credentials and lodge the special consideration application.
Lodge within the window for your assessment type: 48 hours after a centrally conducted exam, two working days after a faculty exam or quiz, or before the due date for an extension.
Keep the practitioner's contact details, because UTS may contact the practice to verify the consultation.

A certificate produced without a consultation does not meet that description. The practitioner also has to supply their AHPRA registration and Medicare provider numbers.
It depends on the assessment. For a centrally conducted exam you attended, apply within 48 hours excluding weekends. For a faculty exam or quiz you attended, apply within two working days. If you cannot attend a faculty exam, apply at least ten working days beforehand, and for an assessment extension apply before the due date.
It is the form the practitioner completes for a UTS special consideration application. It records how severely your ability to complete the assessment was affected, on a scale from totally unable to slightly, and the start and end dates of the impact. It also asks for the AHPRA registration number and the Medicare provider number.
Yes. The UTS Professional Authority Form asks for the practitioner's Medicare provider number as well as their AHPRA practitioner registration number. Check both are present before you upload, because a missing number is a common reason for a request for more information.
Log in to My Student Portal with your UTS credentials and lodge the application there, attaching the Professional Authority Form. Applications are assessed by the faculty that runs the subject, not by a central office.
