This is the main application when illness affects an assignment, test or exam. You lodge it through StudentConnect within three University working days of the due date with the medical certificate attached. The certificate must state the severity of impact, not just that you were unwell.
Where you need more time rather than an adjustment to marking, an extension is the simpler route. A dated certificate stating you could not complete the work on the due date supports the request and covers you if the illness runs longer than expected.
If illness stops you sitting an exam, special consideration can result in a deferred sitting. The certificate needs to show that you could not attempt the exam on the scheduled date, using Deakin's can or cannot complete wording.

See a practitioner while you are unwell. Deakin accepts telehealth consultations, and does not normally accept backdated certificates.
Tell the practitioner which assessment is affected and on what date, so the certificate covers that date specifically.
Ask them to state the severity of impact, moderate or severe, and whether you can or cannot complete the task. Deakin's own certificate form is preferred but any qualified health professional's certificate with that wording works.
Check the certificate carries the consultation date, the period covered, and the practitioner's AHPRA or AASW registration.
Log in to StudentConnect and lodge the special consideration application with the certificate attached, within three University working days of the due date.
