ACU's Special Consideration Procedure does not address consultation modality. Its requirements are about who completes the Professional Authority Section, a registered professional who is not related to you, and that they specify how the condition affected your ability to undertake the assessment.

What ACU asks for
ACU's practitioner form is specific. These are the items the doctor completes after assessing you.
What the same evidence is for
Students search for these by name. ACU treats them as one process with different outcomes.
This is the application when illness or circumstances affected an assessment task. You complete Sections A to C and your professional completes Section D. It is lodged prior to, or up to five working days after, the relevant due date. ACU advises the outcome within ten working days of submission.
Where the issue is time rather than performance, an extension moves the submission date. A dated Professional Authority Certificate covering the due date supports it, and is the same document the full application uses.
If illness kept you from an exam, special consideration can produce a deferred or rescheduled sitting. Section D needs to show the impact fell on the exam date, with the degree of impact rated rather than described loosely.

See a registered professional while you are affected. ACU accepts medical practitioners, psychologists, dentists, physiotherapists and counsellors.
Download the Application for Special Consideration and complete Sections A to C yourself, naming the assessment task and date.
Ask the professional to complete Section D, the Professional Authority Certificate, rating the degree of impact and specifying how the condition affected your ability to undertake the task.
Check Section D carries both consultation dates, the confidentiality tick where relevant, and the professional's registration details and signature.
Lodge the application through the ACU Student Portal prior to, or up to five working days after, the relevant due date.

Silence is not acceptance, so a certificate backed by a real consultation is the safer choice.

It is a combined form. You complete Sections A to C describing your circumstances and the affected assessment, and your registered professional completes Section D, the Professional Authority Certificate, which carries the impact rating and their registration details.
It is Section D of the special consideration form. The professional rates your capacity as totally unable, very severely, moderately or slightly affected, or unable to assess, records two consultation dates, marks whether the condition is confidential, and signs.
