What La Trobe asks for
La Trobe has no practitioner section. The certificate carries the doctor's opinion, so what it states is what decides the outcome.

This is La Trobe's single process for circumstances outside your control that affected an assessment. It is lodged through the online form and assessed on the evidence you attach. The evidence has to link the circumstances to the specific task, not just confirm you were unwell.
Where you need more time to submit, an extension is the outcome the application most often produces. Because La Trobe wants applications before the due date, an extension request is the natural route for an illness that is slowing you down rather than one that stopped you entirely.
If illness kept you from an exam or timed assessment, the application can result in a deferred or rescheduled sitting. Your evidence needs to show the date the circumstances occurred and that the period of impact covers the assessment date.

La Trobe's published policy does not address modality. It requires evidence that is official, verifiable, signed and dated, and that shows how the circumstances affected your ability to complete the task. A certificate meeting those requirements is what the page asks for, whatever the consultation format.
You should submit your application before the assessment due date. Late applications are only considered where you can show exceptional circumstances that prevented a timely submission, so apply as soon as you know the assessment is affected.

Official, verifiable evidence that relates to an allowable reason, shows how your circumstances affected the assessment, and includes the date the circumstances occurred and the period of impact. Documents must be signed, dated, on letterhead where applicable, and in English or professionally translated.
La Trobe publishes processing times separately from the application page. The outcome is normally communicated by the subject coordinator after the application is assessed, so check the processing times page rather than assuming a same-week decision.
