It is a student form. You complete the application yourself and attach medical evidence; there is no section for the doctor to fill in. The in-semester version covers assignments and class tasks, and a separate application covers exams.

What the certificate must state, and what gets it refused.

The practitioner's name, registration or provider number, practice details and signature. For a medical letter of support, that the practitioner has knowledge of your medical history or condition.

The certificate is dated on the day of the consultation and must cover the assessment date, so book before the deadline rather than after it. Monash may contact the practice to verify the document, so keep the practitioner's contact details on the certificate legible.

This is the standard application when illness affects an assignment, quiz or in-semester task. You lodge it through the special consideration application form and attach your medical certificate.
An extension moves the due date rather than changing your mark. Monash treats extensions of up to ten days differently from longer ones: the shorter window is the only place a certificate issued without a video or phone consultation may be considered.
If illness affects a scheduled exam, apply for special consideration for the exam with supporting documentation covering the exam date. Outcomes can include a deferred assessment.
