ANU does not rule telehealth out and lists an online consultation service among the medical options it points students to. What it requires is the medical professional's opinion on how the condition affected your ability to complete the course requirements, which a questionnaire cannot supply.

What ANU asks for
ANU's practitioner form is specific. These are the items the doctor completes after assessing you.
What the same evidence is for
Students search for these by name. ANU treats them as one process with different outcomes.
An Extenuating Circumstances Application, or ECA, is ANU's process for circumstances beyond your control that affected an assessment. You apply within five working days of the due date or the scheduled exam through the Request for Assessment Adjustment page in ANUHub. Medical documentation has to give the practitioner's opinion on the impact.
Where illness stopped you sitting an exam, the ECA is the route to a deferred sitting. The dates of impact on the medical documentation need to cover the scheduled examination date. Apply within five working days of that date.
Where the affected task is not an exam, the same application supports more time. ANU asks for the date the condition began or unexpectedly worsened as well as the impact dates, so an extension can be matched to the period you were affected.

Download the Health Assessment Form from the ANU assessment consideration page before your appointment.
See a doctor while you are unwell and have them complete the form, including their opinion on how the condition affected your ability to complete the course requirements.
Check the document records the date the condition began or unexpectedly worsened as well as the dates of the impact.
Open the Request for Assessment Adjustment page in ANUHub and lodge the Extenuating Circumstances Application.
Attach the documentation and submit within five working days of the assessment due date or the scheduled date of the examination.
