What UC asks for
UC has no practitioner section. The certificate carries the doctor's opinion, so what it states is what decides the outcome.
What the same evidence is for
Students search for these by name. UC treats them as one process with different outcomes.
A deferred assessment is a later sitting of an exam you could not attend. For an unpredictable event such as sudden illness, UC asks for the request no later than three business days after the scheduled exam. For something you know about in advance, apply no later than five working days before it.
An extension moves the submission due date on a non-exam assessment. You request it in writing by email to your Unit Convener, using an Assignment Extension form, and extensions should be applied for before the due date. Where the circumstance was unpredictable, notify the convener as soon as possible and no later than five business days after the due date.
Where a condition is new rather than ongoing, the certificate has to state the impact, the expected duration, and when study and assessment can be recommenced. That last element is what lets the convener set a workable new date. A certificate that only covers the days you were unwell leaves it out.
How to apply at UC
See an AHPRA-registered practitioner while you are unwell. UC will not accept a certificate from a relative, colleague or close friend.
Ask for three things on the certificate: the impact of the condition, its expected duration, and when you can recommence study and assessment.
Check the certificate is on the practice's letterhead and includes contact details.
For an exam, lodge the deferred assessment request no later than three business days after the scheduled exam, or five working days before it if the event was predictable.
For an assignment, email your Unit Convener with an Assignment Extension form, before the due date where you can, and no later than five business days after it otherwise.

For an unpredictable event such as sudden illness, no later than three business days after the scheduled exam. For a predictable event, no later than five working days before the exam. Extensions on non-exam assessments should be requested before the due date, and no later than five business days after it where the circumstance was unpredictable.
Contact your Unit Convener in writing, by email, with an Assignment Extension form. Extensions are applied for before the due date where the circumstance can be foreseen. For an unpredictable circumstance, notify the convener as soon as possible and no later than five business days after the due date.

A practitioner registered with the Australian Health Practitioner Regulation Agency who is not a relative, colleague or close friend with a conflict of interest. Original certificates must be signed by a registered medical, dental or health practitioner. Providing one supports the application but does not guarantee it is approved.
