Silence is not acceptance, so a certificate backed by a real consultation is the safer choice.

It is Federation's practitioner form. The certifying professional records the consultation dates, states the level of impact as severe, moderate, minor or unfit over a specific timeframe, and signs and stamps the form with their provider number. It sits with your application.

This is the application when bereavement, hardship, trauma or medical issues interrupted your studies. It can vary attendance requirements, extend an assessment deadline, or defer an exam.
Where illness prevented you attending an exam, special consideration can produce a deferred sitting. The certification form must state the level of impact over a specific timeframe covering the exam date, signed and stamped by the certifying professional.
