What FedUni asks for
FedUni's practitioner form is specific. These are the items the doctor completes after assessing you.
What the same evidence is for
Students search for these by name. FedUni treats them as one process with different outcomes.
An extension moves an assessment deadline when illness or circumstances delayed your work. Federation runs it as a distinct application from special consideration, with its own page. The Health Care Professional Certification Form supports it where the reason is medical.
This is the application when bereavement, hardship, trauma or medical issues interrupted your studies. It can vary attendance requirements, extend an assessment deadline, or defer an exam. Lodge it within three University working days of the scheduled submission date with the certification form attached.
Where illness prevented you attending an exam, special consideration can produce a deferred sitting. The certification form must state the level of impact over a specific timeframe covering the exam date, signed and stamped by the certifying professional.

See an eligible professional while you are affected: an AHPRA-registered practitioner who can diagnose and treat your condition, a Federation counsellor, or a registered counsellor who is a member of the APS, AASW or ACA.
Bring the Health Care Professional Certification Form and tell them which assessment or exam was affected and on what date.
Ask them to state, in their opinion, whether you were severely, moderately or minor impacted, or unfit, and over which specific timeframe.
Check the form carries the consultation dates, the provider number, the practice stamp and the signature.
Lodge the special consideration application through the Federation student portal no later than three University working days after the scheduled submission date.

Federation's published policy does not address consultation modality. Its requirements are about who certifies and what they state: an AHPRA-registered practitioner who can diagnose and treat, or a registered counsellor, giving a specific impact opinion over a specific timeframe. Silence is not acceptance, so a certificate backed by a real consultation is the safer choice.
