UWA's EVA guidance does not address telehealth or modality. It asks for documentation covering the dates you were impacted, and for a Verification Form completed by a health practitioner once the impact reaches seven days or more.

What UWA asks for

UWA's practitioner form is specific. These are the items the doctor completes after assessing you.

UWA exceptional variation of assessment

EVA is UWA's single process for exceptional circumstances that stop you completing an assessment. It covers assignments you cannot submit within 48 hours of the due date and timed assessments you cannot attend. Apply no later than three calendar days after the due date.

UWA adjustment to an assessment

An adjustment changes how or when you complete the task, for example extra time or a later sit. It is the usual EVA outcome for a short illness that overlaps an assignment or a timed assessment. The documentation needs to cover the dates you were affected.

UWA withdrawal after the academic withdrawal date

Where illness runs long enough that finishing the unit is not realistic, EVA can support a withdrawal after the academic withdrawal date has passed. This is the case most likely to cross the seven day threshold, so plan on providing the EVA Verification Form.

How to apply at UWA

From unwell to lodged, in five steps.

    See a doctor while you are affected, and note which assessments and dates are involved.

    If the impact is under seven days, get a certificate covering the dates you were unwell.

    If the impact reaches seven days or more, including several shorter certificates that add up across the teaching period, take the EVA Verification Form to the consultation for the practitioner to complete.

    Lodge the EVA application through your Student Advising Office no later than three calendar days after the assessment's due date.

    Attach the documentation and, where required, the Verification Form. UWA usually returns an outcome within four days and may ask for a meeting or more documents.

What is UWA EVA?

EVA stands for exceptional variation of assessment. It is UWA's process for when exceptional circumstances stop you completing an assessment or attending a timed one. The outcome can be an adjustment, an exemption or a withdrawal after the academic withdrawal date.

How do I apply for UWA special consideration?

UWA does not use the phrase special consideration for assessment. Apply for an exceptional variation of assessment through your Student Advising Office, no later than three calendar days after the assessment's due date, with documentation covering the dates you were impacted.

When do I need the UWA EVA Verification Form?

When the medical impact reaches seven days or more, either in one application or across several applications that total seven days within a teaching period. Below seven days, an ordinary medical certificate covering the dates is enough.

Does UWA accept online medical certificates?

UWA's EVA guidance does not address telehealth or modality either way. It asks for documentation covering the dates you were impacted and, past seven days, a Verification Form completed by a registered health practitioner. Check with your Student Advising Office if you are unsure.

Does UWA need to know my diagnosis?

No. The EVA Verification Form asks the practitioner to rate the severity of the impact as minor, moderate, severe or unable to assess, and to give onset and end dates. It does not ask them to name the condition.
