What CSU asks for

Special consideration covers illness or misadventure that affected an assessment item. You lodge it through the special consideration application at apps.csu.edu.au and attach the Student Medical Certificate. The certificate has to state the impact of the condition on your ability to complete the subject.
An extension moves the submission date on an assignment. Charles Sturt asks you to apply as soon as it becomes apparent that you will miss the deadline rather than after it passes. A dated certificate naming the period you were affected is the evidence.
If illness stopped you sitting an exam, you can apply for a deferred exam up to three business days after the exam date. The Student Medical Certificate needs to cover the exam date itself. A certificate written days later that does not reach back to the exam is weak evidence.

Download the Charles Sturt Student Medical Certificate before your appointment, so the doctor completes the prescribed form rather than a generic certificate.
See the doctor while you are unwell, and make sure the impact of the condition on your ability to complete the subject is written on the form.
Check the document is legible and signed, with the practitioner's registration details.
Lodge the special consideration application at apps.csu.edu.au/specialcons and attach the certificate.
For a missed exam, lodge within three business days of the exam date. For an assignment, lodge as soon as you know you will miss the deadline.

It is the certificate Charles Sturt prescribes for special consideration. Your medical practitioner completes it and you submit it with the application. Other documentation from a health care provider is only accepted when it contains the same information, including the impact on your ability to complete the subject.
For an exam you could not sit through illness, you have up to three business days after the exam date. For an assignment, lodge the application as soon as it becomes apparent that you will miss the submission deadline rather than waiting until afterwards.

The policy says certificates must be completed in the form prescribed by the University, so the safest approach is to bring the University's form to the consultation.
