What Newcastle asks for
Newcastle has no practitioner section. The certificate carries the doctor's opinion, so what it states is what decides the outcome.

Adverse circumstances is Newcastle's process for illness or other events outside your control that affect an assessment item. You lodge it through the Adverse Circumstances tile in the myUni portal. Applications are usually processed within three university working days and the outcome comes to your student email.
Form A covers an extension of time on an assessment item. Apply no later than 11pm on the due date. A short-term extension of three calendar days or less on an assignment, quiz or in-class test may not need supporting documentation.
Where illness stopped you sitting an exam, apply under adverse circumstances before the exam wherever possible. The certificate needs to name the dates your studies were affected so they cover the exam date. A late application has to explain why it could not be lodged in time.

Log in to the myUni portal, click the Adverse Circumstances tile and follow the prompts.
Lodge no later than 11pm on the due date of the assessment item. If you cannot attach the document yet, lodge anyway and email your Course Coordinator to explain.
Upload the supporting documents through the Adverse Circumstances portal within three calendar days of the online application.
Keep the original document for six months after the end of the term.
