Through MyEnrolment at scu.t1cloud.com. You submit a Special Consideration application form with the accompanying documents attached. Southern Cross does not publish a practitioner form, so a medical certificate is what you attach.
It needs the date on which you were examined and a statement of the dates you were impacted. Because there is no practitioner form, the certificate carries the whole of the medical evidence. Ask the doctor to name the assessment period you were affected by.

Special consideration covers circumstances beyond your control that affected an assessment task. You lodge a Special Consideration application form with the accompanying documents through MyEnrolment.
If you know in advance that you cannot meet a due date, apply at least one day before it. That is a shorter route than a full application after the fact.
Where illness stopped you sitting an exam, the same special consideration application applies. The certificate has to show the date you were examined by the practitioner and the dates you were impacted, and those dates need to cover the exam.
