What USyd asks for
USyd's practitioner form is specific. These are the items the doctor completes after assessing you.
What the same evidence is for
Students search for these by name. USyd treats them as one process with different outcomes.
Special consideration is for illness, injury or misadventure that affected your performance in an assessment you attempted or could not attempt. You apply online and attach the Professional Practitioner Certificate. The outcome depends on the type and format of the assessment, the impact, and fairness to other students.
Special arrangements are for circumstances known in advance, such as a scheduled procedure or a religious commitment that clashes with an exam. You apply before the assessment rather than after it. Medical evidence still needs to name the dates you will be affected.
If special consideration or special arrangements are approved for an exam you could not sit, Sydney may allocate you a replacement exam. Your certificate must show you were unfit on the original exam date. Results can take longer to release after a replacement exam, which may delay re-enrolment.
A simple extension gives you up to five calendar days on an assignment and is arranged directly with the unit of study coordinator. It is an informal arrangement rather than a formal application, and you can only request it in the five days before the due date. Applying for a simple extension does not affect a separate special consideration application.

See a doctor while you are unwell. Sydney prefers an in-person consultation, so use telehealth only where attending in person was impractical, and note the reason.
Ask the doctor to complete the Professional Practitioner Certificate. It asks whether you were totally unable to attend, impacted, or not assessable, and records the dates you were seen.
Check that the certificate names the dates you were affected and describes the impact on your academic performance, not just the diagnosis.
Lodge the special consideration application online through Sydney Student and attach the certificate. The form takes about five to ten minutes and can be saved and finished later.
Track the application through the University's service portal and keep the practitioner's contact details, because Sydney may contact the practice to verify the consultation.

Only in limited circumstances. Sydney accepts in-person consultations, and telehealth consultations where an in-person appointment was impractical. The University states it will not consider online medical certificates issued without a consultation with a doctor.
It is Sydney's own practitioner form. The doctor records the dates you were seen, states whether you were totally unable to attend, impacted, or not assessable, and either names the condition or marks it confidential. You sign a consent section allowing the practitioner to release the information.
Sydney states that deadlines vary depending on your reason for applying, so check the deadline listed for your circumstance before you lodge. Apply as soon as you can after the assessment rather than waiting for the illness to resolve. A late application needs an explanation of why you could not apply in time.
Request it from your unit of study coordinator in the five days before the assignment is due. It covers up to five calendar days and does not need a formal application. It also does not block a later special consideration application for the same assessment.
