JCU states that supporting documentation for medical or psychological reasons must have resulted from a face to face or online consultation with a medical practitioner, and that medical certificates where there has been no consultation, such as one issued from a questionnaire the student submitted, will not be accepted.

What the same evidence is for
Students search for these by name. JCU treats them as one process with different outcomes.
Special consideration covers assessment affected by illness or other extenuating circumstances. For an exam or a timed activity with compulsory attendance, the application is due no later than two University working days after the scheduled date. Attach a certificate that covers that date.
Deferring a formal examination is its own application, lodged with the supporting documentation before the exam or no later than three University working days after it. Use this when illness stopped you sitting rather than affected how you sat.
An extension moves the due date on an assessment item you can still complete. It asks for the same evidence: a certificate from a real consultation showing the dates you could not work on the task.

Book a face to face or online consultation with a doctor. A questionnaire-only certificate is refused at JCU regardless of what it says.
Download the JCU Medical or Mental Health Certificate and take it to the consultation, so the practitioner completes JCU's own fields.
Have the practitioner record the date they examined you, the start and end of the condition, and which activities you could not carry out.
Check the certificate carries the practitioner's signature with letterhead or an official stamp and contact details.
Lodge the special consideration application within two University working days of the exam, or the deferred examination application within three, and attach the certificate.

Complete the application for deferring a formal examination and submit it with your supporting documentation before the exam, or within three University working days of it. A deferred exam is a separate application from special consideration.
JCU asks you to use its Medical or Mental Health Certificate. If it is not used, or not completed correctly, you may be asked to go back to your practitioner for further information, which costs you time against a two day deadline.
It needs the practitioner's name and signature on letterhead or with an official stamp, including contact details. JCU needs to be able to identify and contact the practitioner who examined you.
