What UniSQ asks for
UniSQ has no practitioner section. The certificate carries the doctor's opinion, so what it states is what decides the outcome.
What the same evidence is for

A short extension buys a few days on an assessment item and is requested before the original due date. It is the lightest of UniSQ's options and the fastest to decide. If the illness is likely to run longer, apply for a long extension instead of stacking short ones.
A long extension covers a longer period away from your study and needs supporting documentation. Apply before the published due date for the assessment task and attach a certificate that covers the period you are asking for.
A deferred exam lets you sit the paper at a later date when illness stops you sitting it as scheduled. Apply on or before your exam closing date, and attach a medical certificate or report from a registered practitioner dated on or before the exam.

Lodge the request through the UniSQ Student Centre or from StudyDesk, and attach the certificate for a long extension or deferred exam.
Allow two to five University business days for a decision and keep working on the assessment in the meantime.

Lodge a deferred exam request through the Student Centre on or before your exam closing date, and attach a medical certificate or report from a registered practitioner dated on or before the exam. A certificate dated afterwards will not support the application.
Both short and long extensions are requested before the original published due date for the assessment task. There is no routine after-the-fact extension, so apply as soon as you know you will not finish in time.
Usually two to five University business days when your documentation is complete. Incomplete applications wait, so attach the certificate at the time you apply rather than sending it later.
